How to Apply

Our Family Assistance Fund was created as a way to help employees, their spouses or eligible dependents, who are facing financial hardship because of a qualified incident. An employee can only receive financial assistance once within a 12 month period. If the application for assistance is denied, the employee is eligible to submit a new application (for a different incident) after six months. Eligible dependents may apply for financial assistance on behalf of a deceased employee.

Financial Assistance

Our Family Assistance Fund is intended to help employees that have, or are, experiencing an unexpected qualified incident (i.e. medical expenses, funeral costs, natural disaster, or an unavoidable catastrophic event).

The maximum amount available for each incident is $5,000. Grants may be awarded to help pay for expenses directly related to the qualified incident. In the case of lost wages caused by the qualified incident, our fund may be able to provide financial assistance for regular monthly bills up to the amount lost in wages.

Grants will not be awarded for expenses including, but not limited to: legal fees, insured property losses, insurable medical conditions, bills unrelated to the qualified incident, bills with service dates over 120 days prior to the application date, or non-essential items or services. In certain cases, The Family Assistance Fund cannot pay for items due to lack of home, rental, or medical insurance.

If approved, all Employee Assistance Fund payments will be processed electronically, and within a few days, through the Zelle® interface to your banking institution.

Applications:

Applications to the Fund are reviewed by the Minneapolis Foundation and will be treated in a confidential manner; however non-identifying statistical information will be reported to the employer on a periodic basis. Fully complete applications will be reviewed within five (5) business days. Submitting an incomplete application will delay review.

Applications must be submitted within 120 days of qualifying incident. Employees are encouraged to keep a complete copy of the application for their personal records and to have on hand should we contact you with any questions.
